I am the Owner or Authorized Representative(herein after“ Owner”) of the Jobsite Address at which work is to be done.I hereby authorize Parkerhaus Roofing LLC.to perform said work, and to use such labor, materials and equipment as Parkerhaus Roofing deems advisable.
The repairs performed by Parkerhaus Roofing are based on Parkerhaus Roofing’ s visual inspection and investigation of the area reported problem.Parkerhaus Roofing will not perform infrared or destructive testing or water testing prior to commencing work unless specifically requested, and paid for, by customer.Unless otherwise agreed.Parkerhaus Roofing will only perform a visual inspection, and cannot guarantee that no additional problems will be discovered once repairs begin
hereby authorize Parkerhaus Roofing, LLC to perform the work in accordance with the terms and conditions as set forth above, and represent that I am empowered to authorize service on the premises listed here and I agree to pay according to the Terms.If work to be performed is expected to exceed the limit stated in the Standard or After Business / Weekends as listed a written Estimate is requested in place of this Repair Work Authorization.
I understand and acknowledge that by accepting this work I am providing my electronic signature for Parkerhaus Roofing LLC.to proceed with this work in accordance with any and all applicable laws specifically including, but not limited to, the Federal Electronic Signatures in Global and National Commerce Act and / or Chapter 668, Florida Statutes.
